What to bring to your first appointment at Neurosurgery One
If you are a new patient, please bring these items to your first appointment:
- Completed new patient information forms (download below)
- Any related medical records and all original diagnostic films, such as X-rays, CT or MRI scans. To request that your records be sent to Neurosurgery One, please print and give this form to your referring provider: Neurosurgery One Request for Medical Records
- A list of current medications you are taking
- Your valid insurance card and driver’s license
- Your referral from your primary care physician, if required
Insurance Accepted
Neurosurgery One accepts most major medical insurance plans, but it is important that you verify your coverage. For specific questions about your insurance coverage, please contact our office at 303-734-8650.

Billing Questions
Neurosurgery One services are billed through AdventHealth and Common Spirit. If you have a question about a bill or making a payment, please contact:
- For Neurosurgery One provider charges or hospital charges at Littleton, Lone Tree, Parker or Castle Rock, please call AdventHealth financial services at 855-241-2455. You can pay AdventHealth bills online here.
- For Neurosurgery One provider charges or hospital charges at Lakewood or Westminster, please call Common Spirit financial services at 888-347-3295. You can pay Common Spirit bills online here.
- If you would like to get an estimate for a procedure at an AdventHealth facility (Littleton, Parker and Castle Rock), you can do this online here.
- If you could like to get an estimate for a procedure at a Common Spirit facility (St. Anthony Medical Center and St. Anthony North), go here.
Request for Medical Records
If you would like a copy of your medical records from Neurosurgery One, please download Request for Neurosurgery One Medical Records and submit to our offices (fax numbers can be located on the location page of the clinic where you are seen). The following charges apply:
A patient/personal representative shall pay:
- Postage, plus $14.00 for the first 10 or fewer pages
- Pages 11-40 at $0.50 per additional page
- Pages 41+ at $0.33 per additional page
Note: This fee will be waived if the records are released directly to another healthcare facility or a physician’s office. Records cannot be picked up at our offices; they will be mailed directly to the designated recipient.
Fees for copies of medical records as pursuant to Colorado State Statute 6CCR1011-1 Chapter 2 part 5.2.3.4.
